Assessments and Evaluations:
A Care Manager will meet with the client, family, doctors and friends to gather information on the client's history and identify their mental, physical and emotional capabilities and needs. This process allows us to identify our client's preferences and values which are used as guidelines for serving them. With this information we will develop an individualized care plan that addresses medical, legal residential and personal needs.
Guardianship
Castle Rock is appointed by the court to make life decisions and arrange care for an incapacitated person. The guardian makes decisions about where the protected person will live and assures their medical, social and emotional needs are met.
Care Management
Experienced professionals will assist families, clients, conservators, administrator of trusts and others, in meeting an individual's special needs. Our goal is to help our clients maintain a dignified and safe lifestyle utilizing their values and preferences.
Placement/Housing/Community
Castle Rock staff has extensive knowledge of community resources. We can assist you with placement in a Licensed Family Home, Mental Health Shelter Home, TBI apartment, Assisted Living or Nursing Home. Our goal is to assist our client in choosing the most comfortable, least restrictive living situation.
