Consultation
CRS staff will meet with you to identify and resolve care needs. We are able to help you develop a workable plan, and identify resources to help you make your plan work. Together, we utilize available resources to provide the most effective care.
Assessments and Evaluation
We gather psychosocial histories, medical, and personal information to create an assessment that will determine the care and services you or a loved one may require. Based on this information, our Care Managers consult with you to make an individualized, personal value based care plan.
Guardianship
Castle Rock is appointed by the court to make life decisions and arrange care for an incapacitated person. The guardian makes decisions about where the protected person will live and assures their medical, social and emotional needs are met.
Care Management
Our experienced professionals work with families, clients, conservators, banks and others to meet individual unique needs. Our goal is to help you and your loved one maintain a dignified and safe lifestyle that honors values and lifestyle choices.
Care management services may include regular home visits to assess loved one’s status and needs. CRS
assures doctor’s orders are followed, medications are ordered and support services are in place.
We are there to comfort, assist and advocate for best care during doctor appointments, ER visits and hospitalizations. We work with discharge planners to assure a smooth transition home with homemaking, shopping and support services in place. We keep you up to date about your loved one’s care every step of the way.
Relocation
We will find a living situation that meets your needs. CRS can coordinate your move to be as stress free as possible. We often help families relocate their loved ones in or out of state.
Guardianship
Upon court appointment, CRS arranges care for the protected person (client), based on their life values and their best interest. We work with you and your loved one, whenever possible, to make informed and mutually agreeable decisions about care and placement.
Housing
Castle Rock staff know community resources which give us an advantage in seeking placement into Certified Family Homes, Mental Health Shelter Homes, Traumatic Brain Injury apartments, Retirement, Assisted Living and Nursing Homes. Our goal is to help you and your loved one choose the most comfortable, least restrictive placement.

Castle Rock Services can provide you with a convenient and cost effective way to plan for your health care needs. You can be secure knowing we will be there when you need us, to advocate for you to insure the choices you made will be carried out as planned.
